Community Discharge Assessor

We are looking to appoint a Community Discharge Assessor to work within the Integrated Hospital Discharge providing a seven day service including weekends & bank holidays.
The successful applicant will work with health and social care teams to agree the most appropriate pathway of care to support discharge from hospital.
You must be a Registered Clinical or Social work professional qualification (RN, RMN, AHP, HCPC registration) along with demonstrable experience of contact with patients both in an acute and community setting.
Main duties of the job
The post holder will work collaboratively with the multidisciplinary health and social care teams within the Integrated Discharge Team across acute and community hospitals, and community teams / services to co-ordinate and navigate the patient across pathways to ensure a safe same day discharge for patients deemed fit for discharge.
Primary responsibility to complete Transfer of Care document via a telephone triage system, assessing individual patients care needs for discharge.
Support ward professionals in agreeing the pathway for discharge from Hospital in accordance with agreed policies and professional guidelines.To work flexibly to meet the needs of the service, covering a seven day period including weekends & bank holidays.
The Integrated Discharge Team (IDT) is responsible for ensuring patient flow throughout the Trust is safe and effect at all times.

The IDT encompasses the Health and Social Care Teams, Social Workers, Community Teams as well as Local Authorities and are responsible for ensuring the right patient is discharged at the right time to the right place.
We are a highly motivated and cohesive team putting the patient at the centre of all our discussions. The Integrated Discharge Team is a fast paced area, but ensuring patient safety in the Acute Trust setting is very rewarding.
We pride ourselves on effective communication and team working to support each other and provide the best service we can.
