Job Overview:
An exciting opportunity to join the Thrombosis Service team at Ysbyty Glan Clwyd hospital for a Band 7 thromboprophylaxis specialist nurse (TPSN). The TPSN will be responsible for providing expert advice of venous thrombo-embolism (VTE) prevention to guide practitioners to identify those patients most at risk. The TPSN will be a lead practitioner on raising awareness, education and promoting innovative VTE preventative standards.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

- Take an autonomous role to lead and promote the development and innovation of thrombo-prophylaxis services and work to maintain the exemplary site status of YGC.
- Adhere to the All-Wales Thromboprophylaxis policy; conduct timely root cause analysis on all potential hospital acquired thrombosis (HAT), feedback to clinical teams to ensure lessons learned are applied in practise.
- Identify gaps in service provision and lead on patient safety improvement initiatives.
- Provide expert advice and education on VTE prevention. Act as a resource for training; develop and encourage a champion program for staff.
