Key Responsibilities:

- Nurture excellent relationships with partners especially health partners.
- To ensure the person receives a high quality, strengths based Social Care service, supporting their journey through hospital, returning to the local community
- To ensure the delivery of a responsive and timely service.
- To promote, lead and manage the development of excellent and confident practitioners
- Generous annual leave of 28 days increasing to 33 days after 3 years’ service (pro rata for part time).
