You will be supported in providing leadership, management, and guidance to a multidisciplinary team, ensuring high-quality, strengths-based Social Care services. Additionally, you will play a key role in promoting, leading, and managing the development of skilled and confident practitioners.

Key Responsibilities:

  • To provide leadership, management, and guidance to a team of social workers, support coordinators and therapists including Senior practitioners and Assistant Community Service Managers.
  • Nurture excellent relationships with partners especially health partners.
  • To ensure the person receives a high quality, strengths based Social Care service, supporting their journey through hospital, returning to the local community
  • To ensure the delivery of a responsive and timely service.
  • To promote, lead and manage the development of excellent and confident practitioners
