Job Overview
This is an exciting opportunity for experienced Registered Mental health nurses looking to join the community mental health services in the central area. You will have excellent communication skills, be self-motivated, proactive and committed to the promotion of innovation for service delivery.
The successful candidate will have the skills to deliver holistic, safe, effective care to patients within the community and hold a complex caseload of individuals aged 18-65 with severe and enduring mental health issues and will work collaboratively to assess the needs and create care & treatment plans to meet the identified needs and risks identified. The post holder will be required to work independently and liaise with a variety of agencies and third sector organisations plus the individual’s carers and family members. This post will be initially located within the Community mental health team in Denbighshire however there will be a requirement to work within other teams and this may include inpatient settings in the future.
You will have exceptional communication skills and the ability to build and maintain effective working relationships, demonstrating a ‘can do’ attitude’.
Main duties of the job

The post holder will undertake Care Coordination of service users who have complex mental health needs and will facilitate the promotion and maintenance of a high standard of community psychiatric care, both alone and in conjunction with others from the MDT. This may also involve the post holder being part of a clinic led system for some service users.
The post holder will be expected to work in a recovery focussed way and facilitate user and carer involvement.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

- Evidence of Continued Professional Development

- Broad range of experience working in health & social care settings

- Awareness of current developments within MHLD older peoples service
