Job Overview
The Integrated Community Specialist Palliative Care Team have a rare and exciting opportunity for a Senior Heath Care Assistant to join our team of specialists.
Our specialist HCA's deliver care and support to patients, their carers, and the people important to them.
We are looking for an experienced HCA with an interest and a passion in caring for patients in the community that have complex palliative care and end of life needs.

As a Community Specialist Health Care Assistant, you will need a car, a full UK driving licence, and insurance to cover you using your vehicle for business purposes - our patients live across all of Leicester, Leicestershire and Rutland, therefore you will be required to travel across this whole region. Mileage expenses will be paid.

Job Description Summary (Main Duties And Responsibilities)

- Taking physical or psychological measurements from named clients/service users as delegated by the team.
- Supporting clients/service users in maintaining their health and wellbeing.
- Undertaking specified and delegated clinical and therapeutic activities.
- Under the supervision of healthcare professionals assist the healthcare team with the planning, organising and provision of clinical care.
- Ensuring high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals.
- Assisting registered professionals in the implementation and evaluation of individualised care plans.
- Accurately recording patient information.
- Providing support and supervision to other colleagues and learners as appropriate.
